Referring to Psychologists at Eunoia Minds

Referrals can be made directly to individual psychologists via the contact details below, and this is our preferred method. Alternatively, general referrals can be sent to hello@eunoiaminds.com or faxed to (07) 3740 4751.

If referrals are sent directly to Eunoia Minds, clients details will be passed onto their preferred psychologist.

To ensure referrals and re-referrals are compliant with Medicare requirements and eligible for claiming, please ensure they include a referral letter with the following information. Referrals which do not have all of this information will need to be adjusted before we can claim on them.

• Client's name and date of birth

• Client's medicare number

• A request for services under an existing Mental Health Care Plan, Eating Disorders Management Plan, or Chronic Disease Management Plan

• The reason for referral (i.e., diagnosis eligible under MHCP, EDMP or CDMP)

• The number of sessions requested (up to a maximum of 6 on a mental health care plan, a maximum of 10 on an EDMP, or a maximum of 5 on a Chronic Disease Management Plan)

• The number of sessions requested must not exceed the remaining eligible sessions under the existing plan (e.g., if a client has used 6 of 10 sessions on their MHCP for the year, the next referral must not exceed 4)

• A signature of the referring doctor, and date of referral and signature.
